 Are you a new patient coming to our clinic for the first time?​

In the interest of saving you time, we’ve put our new patient forms online for easy download. Looking over these forms and filling them out as needed will help to streamline your first visit to our practice. Once you’ve kindly taken the time to complete these forms at your convenience, please send them back to us and we will process all your information. This means that when you come in for your first visit, all you’ll have to do is check in and go straight in to see the dentist.

If you’d prefer to complete the forms in the office, or if you have questions about any of the information, please come to your first appointment 15 minutes early. Our staff will be able to assist you in completing the forms.
